IPC: Consent and Waiver

Consent to Rehabilitation and release of liability and waiver or legal rights

I, the undersigned owner or owner agent of the pet identified above, certify that I am over the age of 18 years of age. I hereby consent to my pet participating in rehabilitation at the referenced Facility.

While I accept that all procedures will be done to the best of the abilities of the staff of the Facility, I understand that no guarantee or warranty has been made regarding the results that may be achieved. I fully release the Facility, its officers, directors, owners, employees, agents, contractors, representatives, successors, assignees, and affiliates (collectively, the “Released Parties”) from, and will indemnify and hold them harmless from and against, all liability for any injury, expense and or damage whatsoever arising out of, related to or in connection with my pet’s physical therapy at the Facility.

I further agree that I will not sue or make a claim against the Released Parties as a result of any injury sustained from my pet’s participation in rehabilitation at the Facility. I also agree to indemnify and hold the Released Parties harmless from all claims, judgments, and costs including attorney’s fees, incurred in connection with any action brought as a result of my pet’s participation in rehabilitation at the Facility.
